NJ Children's System of Care, PerformCare is the single point of entry for all children, adolescents, and young adults (up to age 21) who are in need of behavioral health, developmental or intellectual disability treatment services. Depending on your child's unique needs, the system of care can provide supports in the form of:
In-home counseling
Crisis response
Respite services
Care Management
Partial-care placements
Family therapy
Behavioral support

The State of New Jersey and PerformCare work together to administer the publicly-funded developmental disability service delivery system for children and youth up to age 21. In order to receive intellectual and developmental disability services, you must apply for a determination of eligibility this can be done online or by mail.
To access services for children with intellectual or developmental disabilities through CSOC, families must first complete a DD eligibility application. In order to qualify, the child must be under the age of 18 and living with a caregiver. An application is available on the PerformCare NJ website at: https://www.performcarenj.org/families/disability/determination-eligibility.aspx.
