      The State of New Jersey and PerformCare work together to administer the publicly-funded developmental disability service delivery system for children and youth up to age 21. In order to receive intellectual and developmental disability services, you must apply for a determination of eligibility this can be done online or by mail.

      To access services for children with intellectual or developmental disabilities through CSOC, families must first complete a DD eligibility application. In order to qualify, the child must be under the age of 18 and living with a caregiver.
